Social Prescribing
What is Social Prescribing?
Social Prescribing is a free community-based support that aims to reduce social isolation and improve people’s health and wellbeing. Social prescribing is just one response in Irelands' collective response to addressing poor mental health and wellbeing. As an emerging area of practice in mental health promotion, prevention, and primary care provision, social prescribing enables healthcare and other professionals to refer people to a range of local, non-clinical services primarily provided by the voluntary and community sector.Individuals can be referred by a professional or they can make a self-referral to access the service.
How does it work?
Social Prescribing is non-judgmental, confidential and solution focused. Upon receiving a referral, we work collaboratively with participants to identify goals and a plan is put in place to help access activities or services of interest to the individual. The participant chooses activities and supports which are right for them. We follow up with the person to check in on their plan. We work on a one-to-one basis over phone and in person meetings over the course of 3-6 months
Who is it for?
Social prescribing offers a person-centered approach to health and wellbeing for anyone over the age of 18.
